Presenting Complaint

Anorexia and vomiting for 5 days. Vomit material is clear, bile colored.

Physical Examination

  • Depression and lethargy
  • Dehydration
  • Tachycardic (160bpm)
  • Hypersalivation
  • Nasal discharge
  • Mydriasis
  • Decreased anal tone
  • Decreased PLR
  • Elevated nictitans
  • Prolonged Schimer tear test
  • Non-atropine responsive (no increased heart rate)
  • Intradermal histamine - mild flare with minimal wheal
  • Ophthalmic pilocarpine test - meiosis in 17 minutes

Hematological Findings

  • Chloride - 95mmol/L (109-124mmol/L)
  • HCT - 55.7% (37-55%)
  • Monocytes - 2500/ul (100-800/ul)
